Request Letter

The letter below is an example of how to launch a request for accommodation or special education services from a school board or university. This letter attempts to cover all of the basis for later actions for a complaint to the Office of Civil Rights (Department of Education). It is intentionally "open ended" to put the onus on the school or district to fully discover what is needed, especially if the needs change suddenly. This letter initiates a request for action under the following statues, and regulations pursuant to these statues:
on behalf of [Name]. This person has a diagnosis of {Chemical Sensitivity | Myalgic Encepehalomyelitis | Fibromyalgia | Chronic Fatigue Syndrome | Hughes Syndrome} and a physician statement is available to confirm this. This diagnosis has several disabilities known to be associated with it (See Attachment) which may require both evaluation and accommodation. Pending evaluation by qualified individuals who are familiar and experienced with this diagnosis and each of the disabilities associated with it, the following accommodation are requested:
